Carelon Behavioral Health credentialing (Anthem's behavioral health network)

Checked against our own applications on

Short answer

  • For behavioral health, Carelon credentials and contracts for Anthem: commercial, Medicare Advantage and Anthem HealthKeepers Plus (Medicaid). There's no separate Anthem application.
  • Since May 19, 2025, new Carelon enrollments go through Availity, in the Carelon payer space.
  • Credentialing and contracting run at the same time. The contract arrives by e-signature; the credentialing approval comes as a letter by email.

Applying

  1. Make sure your CAQH profile is current, attested and authorizes Carelon/Anthem.
  2. In Availity, open Payer Spaces > Carelon Behavioral Health. The first time, you're asked to accept Carelon's user agreement.
  3. Start a new application in Provider Enrollment for an individual, a group, or a clinician joining a group.
  4. Watch for the contract by e-signature, and check spam. Our practice's arrived five days after Carelon received the application.

Applications started before Carelon moved to Availity don't show on the Availity enrollment dashboard. For those, call.

Calling Carelon

  • Provider services: 1-800-397-1630. Press 4 (all other plans), then 4 (application status).
  • Have the provider ID or application event ID ready.
  • Ask for the effective date, whether your service address is loaded, and which Anthem plans the contract covers.
  • For our individual application, Carelon said it didn't need a separate Disclosure of Ownership form.

Which Blue plan is yours?

In Virginia, Anthem is the Blue Cross Blue Shield plan for most of the state, including Richmond. CareFirst covers Maryland, DC and Northern Virginia; CareFirst members seen elsewhere in Virginia go through Anthem. So for a behavioral health practice in Richmond, Carelon is the route to the Blue plan.
